In accordance with the Lake Land College Academic Integrity Policy, an instructor who has witnessed academic dishonesty will inform the student of the allegation. If the student admits academic dishonesty, the instructor may give the student a reduced or failing grade in the course and will then notify the Vice President for Student Services by submitting this form. According to the Student Code of Conduct and Disciplinary Procedures, the Vice President may impose additional disciplinary penalties. If the student disputes the instructor’s allegation, the instructor should note such on this form and the student will be provided an opportunity for a hearing by the Academic Hearing Committee.
